Canine teeth play a vital role in your smile. They’re key to guiding your bite, supporting your facial structure, and completing the natural curve of your upper arch. But when these teeth don’t come in as expected, it can lead to both functional and cosmetic concerns. What Causes Canine Impaction?

Upper canines are the second most commonly impacted teeth (after wisdom teeth). While they’re designed to emerge during the early teenage years, several factors can interrupt this natural process.

Left untreated, impacted canines may damage nearby roots, contribute to misalignment, or lead to cyst formation. That’s why early detection, typically around ages 11-12, is important.

Recognizing the Signs of an Impacted Canine

In most cases, an impacted canine is first identified during routine orthodontic treatment or a dental exam. However, there are times when certain symptoms signal the need for further evaluation. These may include a delay in the eruption of the adult canine, swelling or tenderness in the upper gum, or the presence of a firm bump beneath the surface.

Some patients also experience bite misalignment or shifting of nearby teeth as a result of the impacted tooth. With the aid of advanced imaging—such as panoramic X-rays or 3D CBCT scans—we can accurately locate the impacted tooth and determine the most appropriate path forward.

Why Early Treatment Matters

The earlier an impacted canine is addressed, the more successful—and less invasive—the outcome tends to be. Treating canine impaction in adolescence allows for greater predictability, faster healing, and a more coordinated effort between the oral surgeon and orthodontist.

Treatment Options for Impacted Canines

In some cases, we may simply monitor the tooth if eruption seems likely to occur on its own. More commonly, treatment involves a collaborative approach with your orthodontist to create space for the tooth and guide it into place.

The most effective and widely used solution is canine exposure surgery, a minor oral surgery that creates a path for the impacted tooth. During this procedure, we carefully uncover the tooth and, depending on the case, may attach a small orthodontic bracket and chain. This allows your orthodontist to gently guide the tooth into position over time.

What to Expect During the Process

Before the procedure, you’ll meet with one of our surgeons for a consultation and imaging review. We’ll discuss sedation options, review your medical history, and answer any questions about the procedure and healing process.

Canine exposure is typically an outpatient procedure performed under local anesthesia with IV sedation for comfort. Most patients are in and out of the office in under 90 minutes. Afterward, we’ll provide a detailed care plan to help you heal comfortably and stay on track with your orthodontic treatment.

Recovery and Aftercare

Healing from canine exposure surgery is generally smooth, especially in younger patients. Some swelling or tenderness is normal in the first few days but can be managed with over-the-counter pain relievers and cold compresses.
